Teaching Administration Form

In order to teach a class at MOVE IT 2012, the teacher or representative from the company must complete this form. You also need to complete this if you are leading a practical workshop at the MOVE IT Teachers' Convention. If you are leading a seminar complete this form >>

Deadline for form completion FRIDAY 3 FEBRUARY 2012

Check your dance class time and description on the class timetable. 

 

Completing the Administration Form
Answer the questions then click next to move forward to the next stage of the form. Make sure you have to hand teacher contact details, risk assessment information and Public Liability insurance details.

Once you have began the form you need to complete all stages to submit. If you complete one stage and leave the form altogether it will not remember where you were up to and you will need to re-enter the information again.
